Afzaal Khan was born in Lucknow, Uttar Pradesh, and completed a Bachelor of Commerce before pursuing acting. He grew up taking part in school dramatics and won several acting competitions, an early interest that eventually prompted a move to Mumbai in the late 1990s to chase a screen career. His family background includes a father who worked in business, and he has a brother, Zohaib Khan, who is also an actor.

Afzaal's first screen work came in the late 1990s with a part in the film Tower House (1999). The commercial outcome of that film did not immediately advance his career, but he continued to seek opportunities in television. In 2001 he auditioned for the popular soap Kyunki Saas Bhi Kabhi Bahu Thi and was cast in a short-lived negative role described as a spoiled brat. That appearance led to further offers, including another Ekta Kapoor production, Kitne Kool Hai Hum, which premiered the same year.

Through the 2000s and 2010s Afzaal built a steady career on television. He appeared in the film Kaash… Hamara Dil Pagal Na Hota (2003) and in TV serials such as Sanskar (2006). He took on multiple episodes of the crime anthology Savdhaan India: Crime Alert in 2012 and played Jagdish in C.I.D. Chhote Heroes (2013). He first joined the long-running procedural C.I.D. in 2007 and continued to portray various characters there until 2014.

He played Munna Mama, a villainous part, in Piya Rangrezz (2015), and was seen in Ek Tha Raja Ek Thi Rani the same year. In 2018 he portrayed Atharv Singh Rathore in Vish Ya Amrit: Sitara. From 2021 through 2023 he was cast as Ram Ahlawat in the daily soap Meet: Badlegi Duniya Ki Reet.
